Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. All responses to this solicitation (RFQ) must make reference to solicitation number F04693-01-Q-0002. Los Angeles Air Force Base (LAAFB) proposes to procure the services of a qualified dietitian to be responsible for providing dietetic health and wellness services for government beneficiaries and DoD civilians that meets the following requirements and specifications: Planning, coordinating, implementing and evaluating Health and Wellness Center (HAWC) and base-wide nutrition and other health and wellness programs. Developing the nutrition budget, creating continuity files, implementing instructional material, aids and techniques. Teaching clients in one-on-one or group settings. Conduct nutrition lectures and workshops on subjects including, but not limited to, smoking cessation programs, fitness programs, drug and alcohol abuse prevention, parenting classes, healthy lifestyle behaviors, minimizing health risks and prevention of further disease complications. Promoting nutrition during health fairs and special theme events. Providing dietary expertise during cholesterol and diabetes screenings and overseeing base-wide weight management program. Responsible for interviewing clients using the Health Enrollment Assessment Review and completing nutrition, health risk education and treatment plans; instructing clients and family members using change model stages on appropriate diet and/or methods to reduce identified health risks. Dietitian will perform a biennial needs assessment to determine base-wide nutritional deficiencies. Provide education based on health risk, high volume and cost effectiveness to clients. Actively market nutrition programs through the newspaper, e-mail, displays, briefings, seminars and workshops. The dietitian will work directly with food facility managers implementing the Check It Out program and advise both the Child Development Center and youth Center nutrition programs. The dietitian shall be available Monday through Friday, 7:30 am to 4:00 pm September 25, 2001 through September 24, 2002, excluding federal holidays. The Government will conduct a criminal background check. A Bachelor of Science in Dietetics or nutrition shall be possessed from an accredited 4-year college or university. In addition, the dietitian shall have completed an accredited dietetic internship. The dietitian must provide a copy of a valid, current, unrestricted state dietetic license and certificate of registration by the American Dietetic Association. Individual must demonstrate dietitian education and training and be certified in Basic Life support (BLS). Individual shall have provided dietetic services a minimum of 1 year in direct patient counseling within the last 3 years. Strong writing and verbal skills must be demonstrated. Certification of current physical examination is required and must be current as of 12 months prior to time of application. Proof of compliance with immunizations (completion of Hepatitis B series, annual influenza vaccination, HIV antibody negative, and immunity to Measles and Rubella A sworn Affidavit of Truthfulness is required, indicating experience, training and technical expertise in the type of care to be rendered. Proposed contract is for a basic year and two option years. Quote submitted should include price for the basic year and each of two option years. Option years will be awarded based on availability of funds. This solicitation is set-aside for Small Business. North American Industry Classification System (NAIC) is 621399. Interested vendors have five (5) days from publication of this Synopsis/RFQ to submit their quote. Anticipated award date is contemplated as 21 Sep 01. The quotation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 97-26. To be eligible to receive an award resulting from this quotation the contractor must be registered with the Central Contractor Registration (CCR) database, NO EXCEPTIONS. Verification of CCR registration and Tax ID number must accompany your quote. The provisions at FAR 52.212-1, "Instructions to Offerors-Commercial Items" and FAR Clause 52.212-2, "Evaluation-Commercial Items", applies to this acquisition and addenda are not attached; FAR 52.212-3, "Offeror Representations and Certifications-Commercial Items" applies to this acquisition and a completed copy of the provision should be provided with the offer. FAR 52.217-5 "Evaluation of Options". The aforementioned addenda are not attached. The evaluation criteria for this solicitation will consist of technical capability of the Offeror to meet the agency technical and delivery requirements, and price.
