Description

Contractor shall provide all supplies, equipment, supervision, personnel and services necessary to perform all examinations as listed in attached CPT/Descriptions for Magnetic Resonance Imaging (MRI), Mammography(Mammo) and after hours Computed Tomographic Scans (CT), Ultra Sound (US) and Nuclear Medicine(Nuc. Med.) imaging studies. This is an indefinite delivery, indefinite quantity contract with a annual minimum of $30,000.00 and a maximum of $120,000.00. RFP-668-312-01 shall be utilized for this specifications. This is an unrestricted solicitation with Hubzone accommidations for small businesses. NAICS Code 622110 Size Standard 25.0 Million. Requests must be made in writing for the specification package. Requests for bids will only be accepted from responsible businesses currently engaged in this type of commerce. A 1-year contract with four(4) one(1) year options to renew. A Firm Fixed Price contract is anticipated. Fax request's for this project are acceptable call (509) 434-7101. Estimated bid opening date is October 5, 2001 at 2pm local time. Project documents are available immediatedly (September 11, 2001). Contractor to provide MRI CT Scamm Mammography Services for the Department of Veterans Affairs Medical Center, 4815 N. Assembly Street, Spokane, WA 99205, in accordance with the terms and conditions herein, for the base period of the Award Date 2001, through September 30, 2002, with four (4) one year options to renew. If all options are exercised the final expiration date will be September 30, 2006.
