Description
This is a combined synopsis for a commercial items prepared in accordance with the format in far subpart 12.6,as supplemented with additional information included in this notice for the South Texas Veterans Health Care System.This announcement constitutes the only solicitation; Request for Quote (RFQ)is being requested and a written solicitation will not be issued. Solicitation documents and incorporated provisions and clauses are those in effect through federal acquisition circular 97-27. This procurement is small business set-aside. NAICS 811219 with business size standard $5M. Prospective vendors are required to submit written proposal for full service on-site service/maintenance for government owned DVI dictation system. The Government anticipates to award a single firm-fixed price contract to the contractor providing the best value for the Government in regards to technical expertise, past performance and price. Services to include parts,labor,equipment,tools and insurance for the period of base year plus 4 option years for renewal, with expected award date NLT October 1, 2001. Quotes must include base year and option year pricing. The Contractor shall support operating software that pertains directly to routines that interface directly with hardware peripheral devices,the computer operations, and applications and utilities programs. Alterations made to system will be made only with the consent of the Government.All maintenance inspections will be scheduled and coordinated with facility. Routine maintenance hours will be from 8:00 to 5pm Monday thru Friday, with 4 hour response time. Nights, weekends, and holidays response time will be 6 hours. Emergency response calls will be no more than four (4) hours. Emergency calls will be determined when 25% or more of system is non-functioning. All service technicians will be required to have certification for service and/or repair from manufacturer for system. ALL OFFERORS ARE TO INCLUDE A COMPLETED COPY OF THE PROVISIONS AT 52.212-3, OFFEROR REPRESENTATIONS AND CERTIFICATIONS COMMERCIAL ITEMS, WITH OFFER.
