- Provide Architect/Engineer (A/E) consulting firm shall provide all investigation, design surveys, verification of as-built conditions, construction drawings, cost estimates, phasing and estimated construction time required for a fully developed set of construction documents for renovation of the 10th floor, Quadrant C, approximately 4880 GSF. The objective of this project will be to provide for the total demolition and rebuild of the Dialysis unit, including all rooms labeled between 10C100 and 10C114, inclusive, but also 10A111 and the C10A1 and C10A5 elevator vestibule and corridor C10B1. The area will be reconstructed to house the Dialysis Unit located on the 10th floor, Quadrant C. Patient Privacy will be given utmost consideration in the design layout. All new mechanical, plumbing, electrical, telecommunication equipment and distribution systems will be provided to modernize and improve the infrastructure supporting the floor's environment of care. All existing windows on the 10th floor, Quadrant C side of the facility will be replaced with new aluminum, side swing, double glazed windows with integral aluminum blinds. The A/E will be required to provide construction documents that will require the project to be phased to accomodate space limitations at the medical center. The A/E may be required to provide the services of a Certified Industrial Hygienist (CIH)to incorporate asbestos abatement plans and specifications into the bid package. The A/E will provide, through the CIH, construction period services to include air monitoring with written reports to establish sample level, air sampling during abatement, and final air clearance samples, and submittal review of required asbestos removal contractor submittals. The A/E will provide a complete set of "as-built" reproducible (mylar) drawings and AutoCAD disks. The North American Industry Classification System Size Standard Code is 541310, Architectural Services, $4.0 million dollars. Standard Forms (SF) 254's and 255's, including each sub-consultant's current SF 254 will be accepted from responsible firms with a 100 mile radius of the New Orleans Metropolitan area only until close of business October 18, 2001. Faxed copies of the SF 254 and 255 will not be accepted. The radius will be determined based on the address stated in block 1 of the SF 254. It is the intent of this agency to interview a maximum of five A/E firms. The A/E must demonstrate his and each sub-consultant's qualifications with respect to the published evaluation factors for design and any optional services. Evaluation factors (1) through (6) are of equal importance; factor (7) is of lessor importance than factors (1) through (6). Specific evaluation factors include: (1) Professional qualifications of the firm and staff including those of any consultant, proposed for designing the project described above; (2) Specialized experience and technical competence in the type of work described above. Firm's prior experience should include, at a minimum, completion of projects associated with health care facilities, to include emergency power installation and code compliance issues where appropriate; (3) Capacity of the firm and project teams to accomplish subject project; (4) Past performance on contracts with Government agencies (emphasis on VA work) and private industry in terms of cost control, quality of work, and compliance with performance schedules; (5) Specific internal quality control procedures. Firm will be evaluated on the acceptability of their internal quality control program used to ensure technical accuracy and discipline coordination of plans and specifications; (6) Location in the general geographical area of the anticipated projects (within 100 of New Orleans) and knowledge of local site conditions; and ability of the firm to provide timely response for on-site support. (7) The volume of work previously awarded to the firm by the Department of Veterans Affairs. Award of contract is subject to availability of funds. The construction range is between $1,000,000.00 and $2,000,000.00.
