Description

This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with the additional information included in this notice. This solicitation number is DADA10-01-T-0282 and is issued as a Request for Quotation (RFQ). This procurement is unrestricted and will be awarded on a full and open basis. The North American Industry Classification (NAIC) Code is 511130 and the Small Business Standard is 500 employees. This is an Indefinite Quantity firm fixed price contract with Economic Price Adjustment (EPA). A task order will be issued for each years' requirement. The anticipated closing date is September 24, 2001. The provision at FAR 52.212-1, Instructions to Offerors-Commercial, without addenda, FAR 52.212-4, Contract Terms and Conditions-Commercial Items without addenda, FAR 52.212-5, Contract Terms and Conditions-Commercial Items without addenda, FAR 52.217-8 Option to Extend Services, 52.217-9, Option to Extend the Term of the Contract, FAR 52.232-18, Availability of Funds, and FAR 52.232-19, Availability of Funds for the Next Fiscal Year, apply to this acquisition.. The contractor shall provide 500 Trauma Nurse Core Course (TNCC) Provider Books and administrative fees and 60 TNCC Instructor Books and administrative fees for a base period (1 Oct 01 through 30 Sep 02) and four 1-year option periods. Offerors shall include a completed copy of the provision of FAR 52.212-3, Offeror Representations and Certifications-Commercial Items with its quote. The Contractor agrees to comply with the following list of DFARS Clauses, which are included in this solicitation by reference to implement provisions of law or Executive Orders applicable to acquisitions of commercial items -- DFARS 252.212-7000, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items; DFARS 252.201-7000, Contracting Officer's Representative; DFARS 252.204-7004, Required Central Contractor Registration. Quotes maybe provided on company letterhead stationery and shall provide this office the following information: Request for Quotation number, company name, address, telephone number, fax number, Cage Code, DUNS number, tax identification or social security number, point of contact, and correct mailing and remittance address. Quotes shall be faxed, mailed or hand carried to MEDCOM Central Contracting Center, ATTN: Maria Fidone, Bldg 4197, 2107 17th Street, Fort Sam Houston, TX 78234-5015 NLT 3:00 p.m. Central Daylight Time on the closing date. All responsible sources may submit a quote which will be considered by the agency.
