Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation to be issued. Written quotations will not be issued and each contractor is expected to respond based on requirements. Request for Quotation, RFQ number 672-200-01 is hereby assigned to this procurement and shall be referenced on all correspondence relating to this acquisition. This RFQ is issued using Simplified Acquisition Procedures and incorporates all provisions and clauses as those in effect through Federal Acquisition Circular (FAC) 97-27 and Veterans Affairs Acquisition Regulation (VAAR) 97-1. North American Industry Classification System (NAICS) 33911 will apply. The San Juan VA Medical Center has a requirement to purchase the following equipment: Item number one: One (1) each Continuos Renal Replacement Therapy Pump System, along with all necessary accessories. Required delivery date is 15 days from date of contract award. Inspection and Acceptance shall be at destination. Contractors shall provide the following information: Terms and condition regarding equipment warranty, Current Freight On Board (FOB) destination pricing, lead time (delivery time), payment terms, Cage Code, Duns/Bradstreet number and Taxpayers I.D. number (TIN). The following provisions and clauses are applicable and are hereby incorporated into the solicitation by reference and will be provided by the Contracting Officer in full text upon request: FAR 52.212-1 (October 2000), Instructions to Offerors Commercial Items applies. Offerors are required to complete and include a copy of the following provisions with quotation; Far 52.212-3 (May 2001), Offeror representation and certifications Commercial items, FAR 52.212-4 (May 2001), Contract terms and conditions Commercial Items, FAR 52.211-5, addendum New Material, FAR 52.219-6, Notice of Small Business Set Aside, FAR 52.212-5, addendum to FAR 52.212-1, Paragraph (b). Offerors shall provide an express warranty which at a minimum shall be the same warranty terms, including offers of extended warranties offered to the general public. Express warranties shall be included in the contract. The clause at 25.212-5 (May -- 2001), Contract Terms and Conditions required to implement statutes or Executive Orders. Commercial Item applies with the following applicable clauses for paragraph (b): FAR 52.222-26, Equal Opportunity, FAR 52.222-35, Affirmative action for handicapped workers, Employment Reports on Special Disabled Veterans of the Vietnam Era. FAR 42.232-18, Availability of Funds (APR 1984). Offerors may find the FAR Provisions at http://www.arnet.gov/. The Government intends to make a single award of a Firm Fixed Price order to the responsible contractor whose offer provides the best market value to the Government based on the following criteria: (1) Overall Pricing factor.
