Description

This is a combined synopsis/RFQ for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are requested and a written solicitation will not be issued. The US Air Force, Luke AFB, AZ. is in the process of upgrading PACS and Teleradiology. This project includes 1 year onsite warranty on parts & labor. Includes 24 x 7 hotline support, Includes 2 hours of instruction on the Referral workbench at agreed upon times. The government is seeking the following commercial items: All items are manufacture specific or equivalent: DICOM Integration fees-CT and US, 2 each; The Roentgen Files Web Server-Linux based DICOM Server/Web Server- No archival, IBM Netfinity Server 5000 Pentium III 1000 MHz, 256 MB memory,18GB mirrored disk, Can be upgraded with additional processor, 100 mb/sec Ethernet cards, 15? Color monitor, UPS, DB-2 database, 1 each; Security Device-Cisco 3005 and certificates Provides encryption and certificates for software and hardware clients at up to T-1 speeds. Requres ports to be opened in firewall ? or can be run in parallel, 1 each: The Referral Workbench-Software only solutions for home PC?s. 12-bit DICOM Viewers Minimum system configuration Windows NT or 2000, 128 MB memory, 1280 x 1024 graphics card and monitor, connection to ISP with Static IP address (one that does not change) 2 each; Cisco 3002-hardware encryption, certificate box for home systems, 2 each; Web viewers ? Software only Solution for home PC?s. 12-bit DICOM viewers, Minimum system configuration Windows 95,98,NT or 2000, 64 MB memory, 1280 x 1024 graphics card and monitor, 56 kb modem (ISDN preferred) Hospital Viewers Software only Solution for home PC?s. 12-bit DICOM viewers, Minimum system configuration Windows 95,98,NT or 2000, 64 MB memory, 1280 x 1024 graphics card and monitor, 56 kb modem (ISDN preferred) =20 Period of Coverage will be Date of Award thru one Calendar year. The government prefers FOB Destination pricing. If quoting FOB Origin, include city/state and estimated shipping cost. The Government will award a Purchase Order resulting from this combined synopsis/RFQ to the responsible contractor whose quotation, conforming to this RFQ, which is the lowest price, and most advantageous to the Government. Offers shall be evaluated for fair and reasonable price, adherence to applicable clauses, provisions, and delivery schedule and satisfactory past performance record. The government anticipates a Firm Fixed Price purchase order. Incorporated provisions and clauses are as follows. FAR Provision 52.212-1, applies to this acquisition. Offer shall include a completed copy of the provision at 52.212-3, Offer Representation and Certification-Commercial Items, which may be accessed on the Internet (http:''www.arnet.gov/far/). Additional applicable FAR Clauses: 52.212-4, 52.212-5, 52.222-3, 52.203-6, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-1, 52.225-3, 52.225-13, 52.225-15, 52.232-33, 52.232-34 and GSAM Clause 552.212-71, 552.229-70 apply.
