- Description
- SCOPE OF WORK PURPOSE: The mission of the Fort Peck Service Unit Indian Health Service (I.H.S.) is to provide the best possible support to the I.H.S. Health Centers through the recruitment and placement of physicians and other health professionals. The purpose of this procurement action will be for the provision of Nephrology Services through the award of a Non-Personal Health Care Contract (Commercial Item). DUTIES/DESCRIPTION: 1. Contractor shall provide on-site nephrology services at the Verne E. Gibbs Health Center. 2. Services will be provided one (1) day per month for eight (8) months out of the fiscal year and two (2) days per month for four (4) months out of the fiscal year. Time of service will be between the hours of 8:00 am to 5:00 pm, inclusive of travel. 3. To provide Nephrology Specialty Clinic services to patients referred by physicians of the Fort Peck Service Unit. The scope of the pre-dialysis renal clinic consists of seeing patients by referral from the staff physicians of the Fort Peck Service Unit when they are found to have difficulty to control hypertension, proteinuria, declining kidney function, or other problems related to the kidneys. Contractor shall provide in-service training on nephrology related disorders to I.H.S. and dialysis unit staff members. Contractor shall make recommendations to the doctors regarding their medical treatment and any planning necessary for dialysis or kidney transplant. 4. Contractor shall be responsible for the submission of medical documentation and evaluations. 5. Contractor shall prepare in a timely fashion the medical and other required records (i.e. patient billing forms and referral forms to other specialists or for other specific diagnostic procedures) for patients whom they provide treatment. 6. Nursing assistance will be provided by I.H.S. for the Specialty Clinic. PERIOD OF PERFORMANCE: Performance of this contract shall be for a twelve (12) month period from October 1, 2001 through September 30, 2002 with an option to renew for two (2) successive twelve (12) month periods. If the government exercises the option pursuant to Federal Acquisition Regulation (FAR) 52.217-9 Option to Extend the Terms of the Contract (MAR 1989), the period of performance shall be extended in accordance with the following schedule: Year Option Period Option Year One (1) To be completed at time of award Option Year Two (2) To be completed at time of award CREDENTIALS: 1. Provide verification of current, unrestricted state license, certification or registration as applicable to practice independently in their professional field. 2. It is the responsibility of the contractor to maintain the appropriate medical licensures and privileges throughout the duration of the contract. 3. One letter of reference 4. Curriculum vitae/resume 5. The selected vendor must apply for medical staff privileges at the Fort Peck Service Unit. 6. If the perspective contractor has already been credentialed, please state so and the Fort Peck Service Unit will verify. INSURANCE: 1. Indemnification and Medical Liability Insurance, FAR 52.237-7. 2. Contracting Officer, Indian Health Service, must obtain evidence of insurance PRIOR TO AWARD (FAR 37.402).
