- Description
- (i)This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in the Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. NO OTHER DOCUMENT WILL BE ISSUED. (ii) This solicitation is issued as Request for Quotation (RFQ) 40804 -- 2G1001.(iii) This solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular (FAC) 97-27, dated August 7, 2001. (iv) This solicitation is set-aside 100% small business and the North American Industry Classification Systems (NAICS) code is 611699. (v)(a.) Service to be Performed: Instruct inmates in the on-site Barbering Program. Classes will consist of hands on/live work, classroom teaching and any other subjects mutually agreed upon by the contractor and the COTR. Due to security restrictions, some teaching methods and/or supplies may be modified by USP Leavenworth's correctional service staff. The instructor must have a valid barber's license and job experience in correctional environment is preferred. The instructor will be responsible for the administration and grading of exams. The instructor will also be responsible for the administration and evaluation of live-work projects. The course must lead to a certificate upon completion of the program. The course must be accredited by an accrediting agency such as Accrediting Commission, Independent Colleges and Schools, North Central Association of Colleges and Schools, or Accrediting Commission for trade and Technical Schools. (b.) Availability of Service: Each class will not exceed 24 inmates. Two (2) sessions per day, Tuesday thru Friday (if required by the Supervisor of Education), from 11:30 am to 3:00pm and 5:00pm to 8:30pm. (c.) Supplies: All equipment, supplies, and materials will be supplied by the Education Department, USP Leavenworth. (d.) Inmate Management and Institution Security: The contractor will adhere to specific rules and regulation of the department and institution. USP Leavenworth education staff shall provide assistance and monitoring procedures will be covered during orientation. Attendance by the inmate participating in the course is mandatory and absentees shall be reported hourly to maintain accountability. Infractions will be reported to the Supervisor of Education. Any individual working within the confines of the US Penitentiary must undergo a background investigation and drug test prior to being permitted inside the institution. In addition, a 3-4 hour orientation class must be completed prior to beginning the contract work. The time spent in this class is reimbursable to the contractor at a rate negotiated with the government at time of the award of the contract. The contractor will not be supervised by the Government, however, work will be monitored in order to verify satisfaction of contract requirements. (vi) Evaluation Criteria: The offers received will be evaluated on price and other factors considered that will be most advantageous to the Government. Offers will be evaluated on (1) Technical/Barber Instructor Experience, (2) Experience in a correctional environment, and (3) Price, with #1 being the highest evaluated factor. The government may award the contract without discussions, therefore, offerors are instructed to submit their best offer initially. A site visit is not scheduled. (vii) Place of Performance: U S Penitentiary, 1300 Metropolitan Avenue, Leavenworth, KS. 66048. Period of Performance: Date of Award until 9/30/2002. THIS IS NOT A PERSONNEL APPOINTMENT. The award of the contract does not constitute an employee/employer relationship. Contractor will not be supervised, but will be monitored to determine if services rendered are in compliance with the contract terms and conditions. Payment will be made upon receipt of an invoice, and after verification that contract terms and conditions have been met. (viii) FAR Provision 52.212-1, Instructions to Offerors-Commercial Items (Oct 2000), is hereby reference. The following addenda are included by reference: At paragraph C, Period of Acceptance of Offers, offerors must agree to hold the prices submitted for sixty (60) calendar days from the date specified for receipt of offers. At paragraph H, Multiple Awards, the Contracting Officer has determined multiple awards are NOT applicable in the solicitation, and the paragraph is hereby removed in its entirety. (ix) FAR Provision 52.212-3, Offeror Representations and Certifications-Commercial Items will be completed prior to award. (x) FAR Provision 52.232-33, Mandatory Information for Electronic Funds Transfer Payment applies to this solicitation. In addition, contractor MUST provide federal tax identification number or social security number PRIOR to award. (xi) Offers will NOT be accepted by fax. Offers must be received in the Contracting Office no later than 2:00pm, Central Time, October 29, 2001. Envelopes must be clearly marked with solicitation number. THIS IS NOT A SEALED BID. Late offers will be rejected, unopened.
