Description

The Veterans Integrated Service Network (VISN) 21 is soliciting proposals to provide Dental Laboratory Services for crowns and bridge units for the Livermore Division. These services will be provided for the VA Palo Alto Health Care System, Livermore Division. Mail proposals to Lupe Arroyo, Contracting Officer, VA Palo Alto Health Care System, 3801 Miranda Avenue 90/CCA, Bldg. 6, Room C-136, Palo Alto, CA 94304. All proposals must be received in the issuing office no later than November 16, 2001. No telephonic request will be accepted. Questions regarding this solicitation shall be submitted in writing to fax number 650-849-0332, no later than November 5, 2001 with NO EXCEPTIONS.
