Description

Base year 01 Casket, standard size, supplies and services in accordance with specifications (appendix J.) In addition in cases wherein the degree of mutilation or dismemberment physically prohibits normal dressing, the casket must have the capability of being hermetically sealed on accordance with specifications. 02 Lightweight shipping container, standard size (casket overpack, air tray, or equivalent) in accordance with specifications (Appendix J). 03 Casket, oversize, supplies and services in accordance with specifications (appendix J.) In addition in cases wherein the degree of mutilation or dismemberment physically prohibits normal dressing, the casket must have the capability of being hermetically sealed on accordance with specifications. 04 Lightweight shipping container, oversize (casket overpack, air tray, or equivalent) in accordance with specifications (Appendix J). 05 Hardwood casket, standard size, supplies and services in accordance with specifications (Appendix J). In addition in cases wherein the degree of mutilation or dismemberment physically prohibits normal dressing, the casket must have the capability of being hermetically sealed on accordance with specifications. 06 Lightweight shipping container, oversized (casket overpack, air tray, or equivalent) in accordance with specifications (Appendix J). 07 Transportation of remains in accordance with specifications (Appendix J) and as provided for in DFARS 237.7002(a), (b), and (c) of the DFARS 252.237-7004 "Area of performance" clause. 08 Cremation, to include embalming, supplies and services; spun bronze urn with carrying container (including engraving) in accordance with specifications (Appendix J). 09 Preparation of remains for cremation and providing a hardwood casket in accordance with specifications. 10 Death Certificates: a. 40 death certificates based on 10 deaths. b. 1 death certificate per death c. 3 additional death certificates per death. Option Year 1 11 Casket, standard size, supplies and services in accordance with specifications (appendix J.) This request for quote for supplies and services will serve the Corpus Christi TX Naval Hospital and is therefore restricted to that area.
