This announcement is published as a Total Small Business Set-Aside action per FAR 52.219-1 and 52.219-6 (this means you must be a small business, quoting the product of a small-business manufacturer); all eligible, responsible sources may submit an offer. This is a combined synopsis / solicitation for commercial items prepared in accordance with FAR 13 and the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitute s the ONLY solicitation; quotes are being requested and A SEPARATE WRITTEN SOLICITATION WILL NOT BE ISSUED. Solicitation number / RFQ N00244-02-T-0017 applies; an electronic copy of the RFQ can be accessed at http://www.neco.navy.mil. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27 and DFAR Change Notices effective through 10/01/01. The standard industrial code is 8661 (NAICS 81311), and the business size standard is $5 million. The agency need is for a Catholic Priest to provide services for Camp Pendleton Naval Hospital in San Diego CA. The contract time period is for one base year and 3 option years, commencing on 11/1/01 through 9/30/02. This requirement is for a fixed priced contract. The statement of work is as follows: STATEMENT OF WORK 1. Contractor Requirements A. The civili an priest shall be an independent contractor of the Pastoral Care Service Department. The contractor shall celebrate 52 Sunday Masses and 6 Masses for Holy Days of Obligation that fall on weekdays per year. The contractor shall also perform no more than 84 other services per year, to include, but not limited to: sacramental acts for patients and staff on days other than Sunday or Holy Days of Obligation; assist the director of religious education with the religious education program (provide guidance, and teaching of sacrament classes as requested); celebrate weekday Masses on an occasional basis as requested by the Department Head of Pastoral Care Services; and conduct patient / staff visitation on Sundays / weekdays as the need may arise. Sacramental acts conducted on the same "on call" requirement / need as determined by the Department Head for Pastoral Care shall be cons idered as one payable service, vice multiple payable services. Patient / staff visitation conducted on the same "on call" requirement / need as determined by the Department Head for Pastoral Care shall be considered as one payable service, vice multiple payable services. B. The term of this contract, the priest, agrees to perform on behalf of the U.S. Government duties of a Roman Catholic priest for spiritual care of active duty military personnel, families, eligible civilian employees, and other beneficiaries per the terms and conditions of the contract. C. Place of Performance -- Work shall be performed for all Sunday Masses and Masses for Holy Days of Obligation during the course of the year. Sunday mass is now scheduled at 0930. Times for Mass on Holy Days of Obligation will be determined by mutual consent between the contractor and the Department Head of Pastoral Care Services. Daily Mass is scheduled at 1130, Monday through Friday. It is normally expected that the contract priest will arrive hour prior to and stay hour after the end of each Mass. Other services will be performed as needed at the discretion / authorization of the Department Head Pastoral Services, with no set length of time set for each performance. Flexible hours during the regular workweek, Monday through Friday, may be employed at the mutual consent of the contractor and the Department Head for Pastoral Care Services. D. Communication -- The contractor is authorized to meet with Directors, Department Heads, supervisors, and other staff personnel as well as patients and their families in the performance of pastoral responsibilities. The contractor will pursue all administrative matters through the Department Head of Pastoral Care Services. E. Personnel Qualifica tions -- The contractor shall be a validly ordained Roman Catholic priest in good standing with his diocese or religious order. The Catholic priest shall demonstrate reliability, good relational skills and the ability to serve on an ecumenical ministry team. The contracted priest will comply with government policies and regulations in regard to identification badges, parking, and professional decorum. F. Guidance -- The Department Head of Pastoral Care Services will provide direct guidance for the contracted priest. G. INS Verification -- No alien shall be allowed to perform under this contract without an Alien Registration Card ("green card") and / or in violation of the Immigration Laws of the United States. H. Personal Appearance -- The contractor shall be neat, clean, well groomed, and in appropriate clergy clothing (shirt with Roman collar) when in patient care and public areas. The contractor shall display the identification badge provided by security any time while at work. I. Government Regulations -- The contractor shall become acquainted with and obey all pertinent Command, Navy, and DOD regulations, shall perform in a manner which precludes any waste of utilities and shall not use government equipment for personal business. All motor vehicles operated on this installation by the contractor shall be registered with base security and operated according to applicable directives. Eating and smoking by the contractor are prohibited in patient care areas and are restricted to designated areas. J. Absences -- The contractor shall advise the Department Head for Pastoral Care Services, or his / her acting substitute, of any planned or unplanned absences. The contractor shall provide a competent priest substitute with equivalent credentials who will provide Catholic coverage during periods of contractor absences. K. The contractor, although in fact not a Government employee, shall comply with Executive Order 11222 of May 8, 1965, "Prescribing Standards of Ethical Conduct for Government Officers and Employees," and shall also comply with Department of Defense (DOD) and Department of Navy (DON) regulations implementing this Executive Order. In addition, the contractor shall adhere to all DOD, Navy, and Command policies and regulations during the performance of their duties. L. As an independent contractor, all medical, dental, and professional liability insurance is the responsibility of the contractor. 2. Property A. Government-Furnished Property -- The government will provide office space, office furniture, telephone / fax service, the use of a copier and computer hardware and software necessary for use by the co ntractor. The government will allow unlimited telephone usage, both local and long distance, in support of the Pastoral Care Service program. The government will provide utility services for the contractor's use while on site. The government will provide referenced information specified in Section 1. The Pastoral Care Service staff will coordinate the retrieval of information needed by the contractor and serve as the interface. The government will provide resources for the administrative and celebration of sacraments and other professional resources necessary by the Department Head of Pastoral Care Service. The government will provide office staff for administrative support. B. Contractor-Furnished Property -- The contractor shall furnish any items, which are for personal use / ownership or which are resources the government may be restricted in appropriating. 3. General Information The following documents and data sources shall be provided to the contractor in the performance of duty upon request. This information will be provided by the NHCP Pastoral Care Services staff: a. Demographic information on the beneficiary population served. b. Current data on patient preferences as determined by survey, correspondence, and patient relations' statistics. c. Information regarding non-medical issues such as facilities, parking, support services, staffing, watch bills, and other amenities. d. Result of inspections / surveys by internal and external agencies. The provision at FAR 52.212-1, Instructions to Offerors -- Commercial Items applies. Addendum to FAR 52.212-1, Paragraph (b)(5): Offers shall provide an express warranty which at a minimum shall be the same warranty terms, including offers of extended warranties, offered to the general publ ic. Express warranties shall be included in the contract. Clause 52.212-4, Contract Terms and Conditions -- Commercial Items applies as well as the following addendum clauses: FAR 219-1, Small Business Program Representations; FAR 52.219-6, Notice of Total Small Business Set-Aside. 52.232-18, Availability of Funds. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items applies with the following applicable clauses for paragraph (b): FAR 52.222-26, Equal Opportunity; FAR 52.222-35, Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era, FAR 52.222-36, Affirmative Action for Workers with Disabilities; FAR 52.222-37, Employment Reports on Disabled Veterans and Veterans of the Vietnam Era.
