Description
Furnish all professional services, travel & incidentals necessary to provide dental services to the students of the Anaconda Job Corps. The Anaconda Job Corps Center is located at 1407 Foster Creek Road,approx. 10 miles West of Anaconda, Montana. The Government will provide the following item(s) of Government Property to the contractor for use in the performance of this contract: Dental examination & work room with normal equipment such as 2 dental chairs with lights & operating unit, X-Ray unit, etc.; Dental supplies, drugs & equipment used in oral examinations, amalgam, silicate, resin, cement restoration, etc. Visits to the Center as mutually scheduled by the Government & contractor to equal 8 hours a week, for the purpose of providing oral examinations, amalgam, silicate, resin & cement restoration; oral prophylaxis; diagnostic radiographs, extractions; repairs of adjustment of prosthetic appliances & those services required below. All above services shall be performed at the Job Corps Center. Visit price shall include cost of dental assistant to be provided by the dentist. Class II American Dental Association (ADA) examination as described by the ADA shall be performed by contractor including but not limited to bite-wing x-rays. Restorative & prophylactic treatment shall be performed on a priority basis. Crown & Bridge Prosthedontics must be approved in advance by the Contracting Officer. All dental services not provided for in this contract will be procured by the Government on a competitive basis to the greatest extent possible. Where practical, price quotes will be solicited from two or more local dental care facilites, including Contractor. Referrals made by the Contractor shall require prior approval of the Contracting Officer. All dental services requiring work off Center or outside contract hours requires prior approval by the Contracting Officer. The Contractor shall provide services which will aid in the development, implementation & monitoring of the total Center Dental Health Program to include, but not be limited to: Advice to the Center Director, Center Physician & the health staff in all dental related matters. This shall include staffing requirements, the advisability of student terminations for dental reasons, the cost of special services & other areas deemed appropriate; Assistance in developing the coordination & integration of the dental health program with the other health components & other Center activity. Contractor shall assist Center Staff to establish dental priorities for a group in order to deliver the most effective overall health care for the budget dollar; The Contractor shall maintain accurate records of patient dental condition. Procedures performed shall be documented on specified Job Corps forms & maintain those records in each individual's student health record at the AJCCCC. Government furnished Standard Forms (SF) 603 and 603A shall be used for such records to be disclosed to the AJCCCC Health Staff & Center Director. Existing enrollee health & dental records will be available to the Contractor. Written prescriptions for drugs & for dental laboratory procedures must be completed as required by law & shall be documented in the enrollee's dental records. Summaries of dental activities shall be reported on the specified Job Corps forms; Advice & assistance to health education instructors in the development & implementation of the dental education component of the Health Education Program (Policy Requirements Handbook (PRH),Chapter 6, Section 3.2; Advice to the Center Director on the Center dental budget including the identification & utilization of the most appropriate & least expensive sources for obtaining necessary dental supplies & medications; Issuance & maintenance of signed & dated Standing Orders for Dental Care in accordance with PRH, Chapter 6, Section 2.3; Dental care instruction shall be provided through the dental health module of the Health Education Program & by the Contractor. The Contractor agrees to adhere to the policies, procedures & guidelines established by the Job Corps in accordance with PRH, Chapter 6, Section 2.7. The Contractor shall have primary responsibility to implement the program outlined above & may be assisted by the Job Corps National Health Staff and/or health consultants. The dental services provided by the Contractor are subject to monitoring & review by the Contracting Officer. Scheduling of appointments will be performed by the Center staff. There may be times when the need for visits to the Center may not exist. In such cases, the Contracting Officer shall notify the Contractor not less than one (1) week in advance of the cancelled visit. The Contract Dentist shall be certified by the State of Montana. Proof of certification shall be required prior to award of contract. Contract Dentist shall be covered by liability insurance in the amount $1,000,000 per occurence, FAR 52.237-7.
