- Description
- Contractor shall provide a complete Radiation Therapy (palliative) program that includes all services mentioned in specification. Follow-up visits are included in, and are a part of, the initial referral fee. No VA authorization (VA Form 10-7078) for the above services will exceed ninety (90)days duration from the beginning date of authorization. Any services beyond (90)days shall require new authorization. All services are subject to the prior approval of appropriate VA medical staff and the Contractor agrees to furnish only those services which are specified and ordered. Contractor shall not bill the patient, the patient=92s private insurance or Medicare for services provided under this contract. The contract price is payment in full. Those patients referred for treatment not paid for by the VA (i.e. private insurance, Medicare, etc) shall be charged the contract price and considered payment in full. No additional charges shall be billed to the patient, the patient=92s private insurance, Medicare or the VA. Billing sent to the patient for informational purposes shall be clearly marked as such. Billing statements shall contain the patient=92s name, social security number, contract price and dates services were rendered. VA and contractor shall mutually agree on a percentage of payment for services rendered if the patient decides to discontinue or becomes too ill to continue treatment.
