Description

Furnish all necessary labor, equipment, material supplies, tools, and transportation to provide Infectious Waste Colle ction and Disposal services for the USPHS-Tuba City Indian Medical Center, Tuba City, Arizona; USPHS-Chinle Comprehensive He althcare Facility, Chinle, Arizona; USPHS-Fort Defiance Indian Hospital, Fort Defiance, Arizona; USPHS-Winslow Health Center , Winslow, Arizona; USPHS-Kayenta Health Center, Kayenta, Arizona; USPHS-Tsaile Health Center, Tsaile, Arizona; and USPHS-In scription House Health Center, Inscription House, Arizona. Requirements type contract, estimate 186,900 lbs/yr of waste to be removed. Services to be provided January 1, 2002 through December 31, 2002 with two (2) 12 month option periods for exte nsion.
