Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented, with the additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number RFQ 667-10-02; award will be made using the simplified acquisition procedures part 13.5. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. This acquisition is unrestricted. The North American Industry Classification System # 421450 and the small business is 500 employees. The acquisition is for the purchase of electrosurgical equipment (APC 300 Argon Plasma Coagulator Unit for Endoscopic applications (ERBA USA Inc.) or equal) and related equipment list below. 1. The quotation shall provide pricing and warranty information on each of the following equipment/supplies: APC 300 Argon Palsma Coagulator, part # 10132-011, 1 each; ICC-200EA w/Endo-Cut and Argon Capable Electrosurgical Generator, part # 10128-205, 1 each; APC Trolley including Accessory Fixation Assembly, part # 20185-009, 1 each; Pressure Reduction Valve, part # 20132-065, 2 each; Argon Gas Tank (Full), 40 cubic foot capacity (approximately 100% pure), part # 1063-2, 2 each; Connection Cable APC 300 to IC-200EA Generator, part # 20192-082, 1 each; Footswitch Connector APC 300, part # 20189-022, 1 each; Reusable Probe Connector Hose 2 meter/6 feet, part # 21132-046, 1 each; Double Pedal Footswitch w/spacebar (waterproof/explosion protected), part # 20189-027, 1 each; Adapter for using Bicap or Goldprobe, part # 20183-053, 1 each; Ignition Tester, part # 20183-060, 1 each; S-Cord 10 feet in length, part # 58301-L10, 1 each; Ultra-8 RF Filter, part # ISOBAR-8, 1 each; APC Grounding Cable Set, part # 7910-2000; 1 each; Flexible APC Probe, diameter 1.5mm, Pulmonary (10 per case), part # 20132-070, 1 case; APC Filter, Membrane Filter (50 per case), part # 20132-059, 1 case; NESSY Monitoring Split Pad w/cable (50 per case), part # 20193-074, 1 case. Delivery is to be made to Chief, A&MMS, Overton Brooks VA Medical Center, 510 East Stoner Avenue, Shreveport, LA 71101. Delivery of equipment is desired between January 2 and February 1, 2002. Inspection and acceptance is at the Overton Brooks VA Medical Center, Shreveport, LA. Quotes shall specify FOB, Overton Brooks VA Medical Center, Shreveport, LA (Destination). If offering an equal product, the vendor must submit documentation of the product with the quote. A company not in possession of FAR 52.212-3 may contact this office or you may access any clause electronically from the above listed internet addresses. Quotes must be in writing and accompanied by a completed Representations and Certifications (FAR 52.212-3) or they will be rejected. Facsimile quotes will be accepted; oral quotes will not be accepted.
