Description

Provide Primary care in Jonesboro, Arkansas. The VAMC Memphis is seeking ofers from medical care providers for primary health care services for enrolled veterans. Payment for services will be on a monthly capitated rate basis. The contract term will be one base year with four-one year options for contract extensions. The primary health care services will be provided in Jonesboro, AR. Faciliites must be in compliance with the American Disabilities Act requirement. The VA currently has 2118 veterans enrolled in a clinic in Jonesboro that will be automatically enrolled with the contractor. The Contractor will provide all personnel, clinic facilites, equipment, supplies, and supervision for the provison of primary health care services. The contrctor will be responsible for the provision of all radiological and laboratory servies associated with primary health care services. The contractor must have a primary care provider on call 24 hours per day, 365 days a year to respond to enroll patient telephone inquires. The VAMC will make an award to the offeror which represents the best value to the VAMC. Contract award will be based upon the evaluation of offers using selection factors including cost as one selection factor. Solicitatiion documents are expected to be available on or about 12/3/01.
