Description

Establishment of a Community Based Outpatient Clinic (CBOC) in Macon, GA. Contractor shall provide Primary Care services at their facility, to beneficiaries of the Carl Vinson Veterans Affairs Medical Center that full fill the VA's healthcare delivery requirements, in accordance with all terms, conditions, and provisions of the solicitation. The contractor shall provide for the continuous delivery and management of primary care for all assigned VA patients. Referrals for specialty care, extensive diagnostic work-ups and non-emergency hospitalization shall be made to the VA. The contractor will be expected to subcontract for any required service, that it does not provide, at no additional cost to the Government. It is the VA's intention to solicit proposals on a competitive basis and award a contract that provides for an "All Inclusive" captiated rate (per member, per month)method of payment. Veterans receiving services under this contract reside primarily in the catchment area of Macon, GA. The CBOC Contractor's facility must be physically located within the following zip codes: 31204, 31206, 31210, 31217, 31220, & 31211. The contract includes a Base Year (one year from date of award) plus 4 one-year renewal options that may be exercised by the Government. The total duration of the contract will not exceed 5 years. This solicitation is issued as a commercial item procurement in accordance with FAR Part 12 and shall be evaluated in accordance with FAR Part 15. This is an unrestricted solicition and is advertised as full and open competition. The NAICS is 621111 and the size standard is $7.5 Million. The solicitation will be issued o/a 11/30/01.
