Q-COMPLETE HOME OXYGEN SERVICE Solicitation RFP-463-0003-02 The Alaska Department of Veteran Affairs Healthcare System and Regional Office (VA) located in Anchorage Alaska is soliciting proposals for Complete Home Oxygen Services . Service shall be for a base year with four (4) one-year options for renewal. Anticipated base year to be 1/1/02 through 12/31/02. The NAICS Code for this procurement is 621610 with a small business size standard of $10.0 million. The Solicitation will be issued on or about December 5, 2001 with proposals due 12/19/01. Evaluation factors will be: 1) Technical capability to meet the government requirements, 2) Experience and Staffing, 3) Range of Services, 4) Past Performance, 5) Price. Requests for solicitation package must be in WRITING, either by mail to Contracting Officer (90C), 2529 DeBarr Road, Anchorage, AK 99508 or FAX (907) 257-6711. Estimated dollar value 1.0 to 2.0 million. Requests for bids will only be accepted from responsible businesses currently engaged in this type of commerce. A Firm Fixed Price contract is anticipated. This is an indefinite delivery, indefinite quantity contract.
