Description

This procurement is 100 percent set aside for small business concerns, Note 1 applies. This solicitation is for commercial items/services prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this announcement. This procurement is conducted under simplified acquisition procedures in accordance with FAR Part 13 and 15. The solicitation documents will be available in the FedBiz web site on or about December 21, 2001. RFQ 546-19-02 should be referenced on all offers/quotes and correspondence. The North American Industry Classification for this procurement is 421450; small business size standard is 100 employees. The Government intends to award a contract for a base year with the option to exercise four (4) one year additional options. The Base year will commence on the award effective date, for a period of 12 months. The Option year(s) when exercised, will commence at the end of each one year period, not to exceed a total of 5 years. The VA Medical Center in Miami, Florida has a need for the following kinds of Sterile Custom made Surgical Packs: 1) Arthroscopy; 2) Cardiovascular; 3) Carotid; 4) Catarac; 5) Ear-Ent; 6) Fistula; 7) Hernia; 8) Nasal; 9) Neuro; 10) Prostatectomy; 11)Total Ortho; 12) Open Heart. A LISTING OF THE CONTENTS OF EACH SURGICAL PACK, presently used by the VA Miami, will be provided with the solicitation.
