Description

This is a solicitation notice for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12.6. Solicitation number Request for Quotation (RFQ) 673-39-02 is assigned and shall be referenced on any request for solicitation. Solicitation shall be issued on or about December 13, 2001. The James A. Haley Veterans Hospital, 13000 Bruce B. Downs Blvd, Tampa, FL is seeking complete maintenance and repair services at the Orlando VA Healthcare Center, 5201 Raymond Street, Orlando, FL 32803. The contract period is January 1, 2002 through September 30, 2002, with two (2) full one-year options for renewal. Contractor shall provide all parts, labor, travel, unlimited service calls and software on an ADAC Gamma Camera Vertex w/Semi Auto Exchanger, Model V60, s/n V009079. This shall include (2) Preventive Maintenance Inspections each year. The contract will allow VA personnel first response to equipment failure. If VA personnel cannot repair the failure, the contractor shall respond by telephone within 2 hours. A service representative shall be on site within 8 working hours, and restore equipment to full performance within 48 hours of the original service call. The North American Industry Classification System code is 811219. The Small Business Size Standard is $5.0 million. All requests for solicitation shall be in writing or via fax at 813-903-4838. The Government shall not be held responsible for the quality of faxes. Pending contract is subject to the availibility of funds.
