- Description
- The Federal Bureau of Prisons intends to award a Non-Personal services contract for radiology services by a licensed radiologist to provide services to the inmate population at the Federal Correctional Institution, FCI Phoenix, located at 37900 45th Ave., Dept 1680, Phoenix, AZ 85027-7000. All services provided shall be performed in accordance with standards and methods generally accepted within the contractor's field of expertise, including but not limited to interpretation of x-rays; providing written and/or dictated reports within five (5) working days after reading of the X-Rays; and will include recommendations for further diagnostic testing and/or treatment. All recommendations will be made to the Clinical Director, who will retain professional responsibility for patients referred. The Offeror must be a certified Doctor of Medicine/Doctor of Osteopathic Medicine degree, with specialty training and certification in Radiology. The contractor must possess certification to practice in this geographic area. Specific services shall be conveniently available to meet the needs of patients as determined by the medical staff, e.g., emergency X-Ray reading. In this case, the X-Ray will be delivered to the Radiologist's office for a separate fee. Due to the nature of this contract, all individuals working within the confines of the institution will be required to submit security clearances. Services will be required during the normal work week, with a maximum of 1 session per week ( 1 session =3D 3 hours), between the hours of 7:30AM to 4:00PM every week (excluding federal holidays). All offerors are advised that their quotation shall be evaluated in accordance with the evaluation factors as cited in the Request for Quotation. The anticipated period of this contract performance will be: the base year from Date of Award, through 09/30/02; Option Period One: 10/01/02-09/30/03; Option Period two: 10/01/03-09/30/04; Option Period three: 10/01/04-9/30/05; and Option period four: 10/01/05-09/30/06. This has been set aside for small business. It is anticipated that this solicitation will result in a Firm-Fixed Requirements type contract. Solicitation will be available O/A 12/21/01 and will be distributed through the General Services Administration's Federal Business Opportunities web site ?www.fedbizopps.gov'. Hard copies of the solicitation document will not be available except in extreme circumstances and only if issuance in this manner is determined to be in the Government's best interest.
